Membership is valid for the duration of the package purchased. Members may choose to
renew their
membership after the expiration date.

Members acknowledge that they are healthy, physically fit, and mentally capable of
performing
athletic activities.

Members acknowledge that it is their responsibility to communicate to the Brawlers' Lab
Instructor/Management of any physical, medical, or psychological concerns that may
conflict with
their participation in athletic activities. Members are aware that they may be directed
to stop by the
Brawlers' Lab Instructor/Management if they display any signs of distress.

Members are aware that the Brawlers' Lab classes and activities are at times combative
in nature,
posing inherent dangers and risks, including accidents, damage to personal items,
injury, illness,
or death. Brawlers' Lab shall not be held liable for these accidents, damages, injuries,
illnesses, or
deaths.

Members will conduct themselves in a quiet, well-mannered behavior at all times.

The management reserves the right to refuse entry and terminate memberships with
immediate effect.
Members expelled will forfeit all privileges of membership.

Brawlers' Lab shall not be responsible for the personal belongings of the members or
visitors.

Brawlers' Lab shall be entitled to make alterations, carry out repairs or maintenance
without
prior notice, though every consideration will be made to minimize inconvenience.

Brawlers' Lab reserves the right to change or amend rules, terms and conditions, fees,
and payments
without prior notice for the good and benefit of the club.

Brawlers' Lab reserves the right to move the facility within Valenzuela area without
prior notice
for the good and benefit of the club.

No refunds or transfers for non-usage of Brawlers' Lab facilities and non-attendance of
Brawlers'
Lab classes/programs.
DISCLAIMER: I hereby state that I have read, understood, and answered
honestly the membership form.
I also state that I wish to participate in all the activities provided by Brawlers' Lab and
hereby
waive release and forever discharge Brawlers' Lab from any and all responsibility or
liability for
injuries and damages resulting in my participation in any activities or my use of equipment
or
facilities in the above-mentioned activities.
